New publication examines diagnostic considerations for a sight-threatening autoimmune eye disease
Andrew Huang, MD, MPH, of the WashU Medicine John F. Hardesty, MD, Department of Ophthalmology & Visual Sciences, recently published a perspective article titled, “Conjunctival Biopsy in Ocular Mucous Membrane Pemphigoid,” examining the evolving role of conjunctival biopsy in the diagnosis and management of ocular mucous membrane pemphigoid (oMMP).
Ocular mucous membrane pemphigoid, formerly known as ocular cicatricial pemphigoid, is a chronic autoimmune disorder characterized by progressive conjunctival scarring that can lead to severe ocular surface disease and vision loss. Early and accurate diagnosis is critical to preserving vision, but identifying the disease can be challenging because its symptoms often overlap with those of other inflammatory and scarring conditions affecting the eye.
In the publication, Huang discusses the traditional role of conjunctival biopsy as a key diagnostic tool for confirming oMMP. However, the article highlights important limitations of biopsy-based diagnosis and emphasizes that a comprehensive clinical evaluation remains essential.
The perspective notes that when a thorough patient history and clinical examination effectively rule out more common causes of conjunctival scarring, the presence of progressive conjunctival fibrosis may be sufficient to establish a diagnosis and begin treatment. In these cases, delaying therapy while awaiting biopsy confirmation could potentially postpone necessary interventions aimed at preventing further vision-threatening damage.
Huang also outlines situations in which conjunctival biopsy remains particularly valuable, including atypical presentations and cases where clinicians must distinguish oMMP from other causes of cicatricial conjunctivitis, such as drug-induced conjunctival scarring.
The article underscores the importance of a multidisciplinary approach to diagnosing and managing cicatricial conjunctivitis, integrating clinical expertise, pathology findings, and patient-specific considerations. By examining the practical applications and limitations of conjunctival biopsy, the publication contributes to ongoing efforts to improve diagnostic accuracy and support timely treatment for patients with this rare but potentially blinding disease.
